AFFIDAVIT FOR LOSS OF DOCUMENT

I, Ramesh Kumar Sharma, son of Mohan Lal Sharma, aged about 35 years, residing at House no., street, area, city, state - PIN, do hereby solemnly affirm and declare as under:

1. That my original Class 10 marksheet (Roll No. 123456) was lost on 15 August 2026 while travelling by bus.

2. That I have searched for it at all possible places but it could not be found. The loss was reported to the police: Lost report No. 45/2026.

3. That the said document has not been pledged, sold or given to anyone.

4. That if the original is found, I shall return the duplicate copy to the issuing authority.

5. That I request the issue of a duplicate Class 10 marksheet.

VERIFICATION

Verified at City on 29/09/2026 that the contents of the above affidavit are true and correct to the best of my knowledge and belief, and nothing material has been concealed therefrom.
